District Nursing:
The District Nurses care for patients who are housebound in the patient’s own home. They work alongside Health Care Assistants, providing nursing care and advice on a range of health care issues, such as:
wound care nutrition pain and symptom control specialist home equipment health promotion care of the terminally ill support for carers.
With patients' consent they work closely with other agencies/professionals, for example, Social Services, Macmillan Nurses, physiotherapists, etc.
Patients are unable to refer directly to the District Nursing Service.
